Not sure how widely known this is, but outside of the obvious Protect And Survive stuff, this is the most unpleasant PIF I've seen:

...Sorry for any nightmares. And for you having to sit through a 15 second long Windows Movie Maker intro. (Seriously though, this is nasty, and real-life medical footage is used...)
